New School Medication Authorization Form

At Blount County Schools, the health and safety of our students is always a top priority. As we prepare for the upcoming school year, we want to make families aware of an important update from the Alabama State Department of Education.

A revised School Medication Prescriber/Parent Authorization Form is now available for any student who needs medication administered during the school day for the 2026–2027 school year.

To help ensure there are no delays in your child's care, we encourage parents and guardians to complete the new form with their child's healthcare provider, and make an appointment with the school nurse before the first day of school.

Please remember:

  • Both the prescribing healthcare provider and parent/guardian must complete and sign the authorization form.
  • Prescription medications must be properly labeled and match the medication Prescriber/Parent Authorization Form.
  • Over-the-counter medications must be in their original, unopened container and meet district and state guidelines.
